Life Events - Health & Welfare Fund
If you get married and would like to add your spouse to your plan, you’ll first need to check your plan’s eligibility rules to see if your plan allows for spousal coverage. If so, you must fill out a new enrollment form and return it, along with a copy of your marriage certificate, to your employer within 60 days of the date of your marriage. Your employer is responsible for submitting your enrollment form to the SEIU Benefit Funds, but we recommend keeping a copy for your records. Your employer may require a higher payroll deduction to add a spouse to your plan.
If you get divorced, contact your employer to update your records. Your employer is responsible for submitting this change to the Fund. If your ex-spouse was covered under your health plan, he or she is entitled to COBRA benefits and the Fund will send him or her this information once we receive notice of the divorce.
If you have a child(ren) and would like to add your child(ren) to your plan, you’ll first need to check your plan’s eligibility rules. If so, you must fill out a new enrollment form and return it to your employer within 60 days of your child(ren)’s birth. Your employer is responsible for submitting your enrollment form to the SEIU Benefit Funds, but we recommend keeping a copy for your records. Your employer may require a higher payroll deduction to add child(ren) to your plan.
If your children are eligible for coverage under your plan, they are eligible to remain covered until the last day of the month in which they turn age 26. They may be eligible to receive COBRA coverage by making self-payments. The Fund will send information about the COBRA option.
If you stop working in covered employment, your health coverage will end on the last day of the month in which your employment ends. Once the Employer notifies the Fund of termination date, the Fund will then send you information on your eligibility to elect COBRA coverage. If you would like to elect COBRA coverage, you must submit the required paperwork within 45 days of the date your COBRA notice.
